Duties
- Act as the main liaison between clients, healthcare providers, and community resources to ensure comprehensive care coordination
- Manage patient intake processes, including collecting medical histories and verifying insurance information in compliance with Medicaid regulations
- Utilize Electronic Health Records (EHR) systems to document medical records, treatment plans, discharge summaries, and case notes accurately
- Assist with discharge planning by coordinating follow-up appointments, community services, and medication management
- Apply medical coding expertise using CPT (Current Procedural Terminology), ICD-9, ICD-10 codes to ensure accurate billing and documentation
- Support behavioral health initiatives by working with individuals with co-occurring disorders and mental health conditions
- Ensure compliance with HIPAA regulations by safeguarding patient information and maintaining confidentiality at all times
Requirements
- Proven experience in hospital or acute care settings with a strong understanding of medical terminology and health insurance processes
- Knowledge of ICD-9/ICD-10 coding systems and CPT coding for accurate documentation and billing
- Background in social work or case management within behavioral health or pediatrics populations
- Familiarity with Medicaid regulations, discharge planning, and working with underserved communities
- Experience managing Electronic Health Records (EHR) systems and electronic health records (EHR) management practices
- Excellent patient service skills with the ability to work compassionately with individuals experiencing mental health or co-occurring disorders
- Ability to handle sensitive information in accordance with HIPAA standards and maintain strict confidentiality
- Strong organizational skills to manage multiple cases efficiently in a fast-paced environment
