Develop, implement, and oversee a resident-centered activity program that meets CMS and Wisconsin DHS requirements.
Complete activity assessments and contribute to interdisciplinary care plans based on residents’ interests, abilities, and needs.
Plan and facilitate individual, group, and one-on-one activities, including social, recreational, educational, cultural, and spiritual programs.
Ensure activities accommodate residents with cognitive, physical, and sensory impairments.
Maintain accurate documentation in residents’ medical records, including assessments, care plans, progress notes, and attendance records.
Coordinate special events, holiday celebrations, and community outings as appropriate.
Recruit, train, supervise, and schedule activity staff and volunteers.
Collaborate with nursing, therapy, social services, dietary, and other departments to support resident well-being.
Manage the activities budget, supplies, and equipment.
Ensure compliance with infection control, safety, and resident rights policies.
Participate in care plan meetings, quality assurance activities, and surveys as needed.
Serve as a positive representative of the facility to residents, families, and the community.
