Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs, reporting changes, and maintaining detailed medical documentation
- Support medication administration in accordance with medical instructions and HIPAA guidelines
- Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques
- Assist with catheter care, wound care, hospice support, and other complex medical procedures as trained
- Use Electronic Health Record (EHR) systems to document patient interactions accurately and securely
- Prepare nutritious meals and assist with light housekeeping tasks like cleaning and laundry to maintain a safe and comfortable environment
- Operate mobility aids such as Hoyer lifts or other assistive devices safely during patient transfers
- Support clients with developmental disabilities or those requiring long-term care by implementing personalized care plans
- Ensure safety protocols are followed at all times during patient visits, including first aid and basic life support procedures when necessary
Qualifications
- Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care settings
- Strong knowledge of medical terminology, medication administration, and home health regulations
- Experience caring for geriatric patients or individuals with dementia, Alzheimer's disease, or disabilities
- Ability to observe patient conditions carefully and communicate effectively with healthcare teams
- Familiarity with EMR/EHR systems for accurate medical documentation
- Certification in CPR and Basic Life Support (BLS) is required; additional certifications such as hospice or hospice care training are a plus
- Physical ability to perform heavy lifting and assist with patient transfers using Hoyer lifts or similar equipment
- Compassionate caregiver with excellent communication skills and a positive attitude toward patient-centered care
- Valid driver’s license and reliable transportation preferred for travel between client locations

Pay: $20.00 - $22.00 per hour
