People Care is an established, licensed home care agency (LHCSA) that has cared for New Yorkers for 50 years. We are hiring RN Field Nursing Supervisors to assess patients in their homes, build their plans of care, and supervise the aides who care for them every day. We are hiring urgently for two full-time positions.
This is hands-on, field-based nursing where your judgment directly shapes how well someone is cared for in their own home. You will report to our Director of Patient Services and be the clinical presence our patients, families, and aides count on.
What you'll do:
Assess patients in the home - comprehensive nursing assessments and reassessments.
Own the plan of care - develop, implement, and update each patient's care plan based on physician orders and condition.
Supervise aides - conduct in-home supervisory visits of home health aides and personal care aides, evaluate competency against the plan of care, and coach in the moment.
Document accurately and on time - assessments, supervisory notes, and care-plan updates in our electronic system, to NYSDOH home care standards.
Coordinate care - work with our coordination team, physicians, patients, and families; catch changes in condition early and escalate appropriately.
Manage your field caseload - prioritize urgent visits and provide reliable, professional coverage across all five NYC boroughs, with Nassau/Westchester field visits as needed.
Required:
- Active, unrestricted New York State RN license in good standing.
- Strong physical assessment and care-planning skills.
- Ability to travel to patients' homes across the assigned boroughs/county; reliable transportation; comfort with NYC transit as applicable.
- Solid clinical documentation skills and comfort with electronic records.
- Current BLS/CPR certification.
- Professionalism, reliability, and the ability to manage a field schedule independently.
Strongly preferred:
- 1 year of home care, CHHA, LHCSA, or community health nursing experience.
- Experience conducting supervisory visits and aide competency evaluations.
- Familiarity with NYSDOH home care regulations, NHTD, CDPAP, and Medicaid managed long-term care.
- Experience with HHAeXchange or a comparable EVV home care platform.
- BSN.
- Bilingual (Spanish, Russian, Creole, or Mandarin) a plus, not required.
