Hospice RN Case Manager (Full-Time)
Location: Riverside, CA and surrounding service areas

Position Details
- Position: Full-Time
- Compensation: Rate discussed during the interview
- Start Date: Able to start ASAP
- Service Area: Riverside and surrounding cities
- Travel: Must be willing and able to drive throughout the agency's coverage area.
Qualifications
- Current, unrestricted California Registered Nurse (RN) license.
- Minimum of 2 years of recent hospice experience as a Hospice Visiting Nurse or Hospice RN Case Manager.
- Current BLS/CPR certification.
- Valid California driver's license, reliable transportation, and current automobile insurance.
- Knowledge of hospice regulations, CDPH requirements, Medicare Conditions of Participation, and hospice documentation standards.
- Proficiency with EMR documentation and strong clinical assessment, communication, organizational, and time management skills.
- Ability to work independently while collaborating effectively with the interdisciplinary team.
Essential Duties & Responsibilities
- Provide skilled nursing care and case management to hospice patients in accordance with the individualized plan of care.
- Complete comprehensive assessments, monitor patient status, and coordinate care with physicians and the interdisciplinary team.
- Provide pain and symptom management while educating and supporting patients, families, and caregivers.
- Maintain accurate, timely, and compliant clinical documentation for all patient care and coordination activities.
- Participate in Interdisciplinary Group (IDG) meetings and contribute to ongoing care planning.
- Supervise Hospice Aides and LVNs as assigned.
- Maintain compliance with all applicable agency policies, CDPH regulations, Medicare Conditions of Participation, HIPAA, and quality standards.
