In this role, you'll help ensure individuals receiving Supported Living Services (SLS) receive our Gold Standard of Care through person-centered planning, strong care coordination, caregiver support, and collaborative relationships with clients, families, Regional Centers, and community partners.
In addition to salary wages, Coordinators are eligible for quarterly growth bonuses!
Schedule
Full-time, Monday–Friday, 8:30 AM–4:30 PM. Participation in the coordination team's rotating after-hours on-call schedule is required.
This position supports clients in Lake and Mendocino Counties and requires regular travel between both service areas. Coordinators should expect to spend at least two days per week in each county, with the remaining time and schedule flexed based on client needs, meetings, caseload priorities, and effective time management.
What You'll Do
Client Care & SLS Coordination
- Manage and coordinate SLS client care using a person-centered approach focused on quality, health, safety, independence, and client goals.
- Develop, implement, and maintain Individual Support Plans (ISPs) and client care plans.
- Complete required ISPs, Quarterly and Annual Reports, Addendums, MARs, emergency drills, medical information, and other client documentation accurately and on time.
- Conduct regular client visits and monitor progress, changing needs, service quality, and overall client satisfaction.
- Coordinate services identified within each client's ISP and collaborate with Regional Center Service Coordinators, families, healthcare providers, outside agencies, and internal teams.
- Maintain accurate and timely documentation and ensure services comply with company policies, Regional Center requirements, Title 17/Title 22, and other applicable requirements.
- Recognize, document, escalate, and report incidents or health and safety concerns, including required Serious Incident Reports (SIRs) and mandated reporting.
- Support medication-related care coordination, including maintaining accurate MARs and ensuring caregivers understand applicable client-specific medication supports.
- Participate in the rotating on-call schedule to assist with urgent client care and staffing needs.
Caregiver Support
- Participate in the hiring, development, training, and supervision of caregivers supporting assigned clients.
- Partner with Scheduling Coordinators to build reliable, appropriately trained client care teams.
- Ensure caregivers understand each client's care plan, goals, routines, behavioral supports, documentation requirements, and health and safety needs.
- Provide ongoing coaching, feedback, training, conflict resolution, and performance support.
- Identify opportunities for performance improvement and collaborate with leadership to strengthen client services and caregiver success.
Client Growth & Community Relationships
- Build strong relationships with Regional Centers, Service Coordinators, families, community partners, and other referral sources.
- Participate in marketing, networking, outreach, and client recruitment activities.
- Communicate our services to prospective clients and families and help them understand available care options.
- Conduct needs assessments and collaborate with the coordination and scheduling teams to successfully onboard new clients.
- Maintain communication with prospective clients and referral partners throughout the intake process.
- Support organizational growth by delivering exceptional client experiences and developing trusted community relationships.
What We're Looking For
- High School Diploma or equivalent required; degree in Social Work, Human Services, Healthcare Management, Business, or related field is helpful, not required.
- At least 2 years of experience in case management, client coordination, direct care, developmental services, mental health, and/or client recruitment preferred.
- Experience supporting individuals with intellectual/developmental disabilities and/or challenging behaviors strongly preferred.
- Knowledge of Regional Center services, IPPs/ISPs, Title 17, Title 22, and Department of Social Services requirements is highly valued.
- Strong communication, leadership, conflict-resolution, organizational, and time-management skills.
- Ability to work independently while collaborating effectively with clients, families, caregivers, and an interdisciplinary team.
- Proficiency with Google Workspace preferred.
- Valid California Driver's License, vehicle insurance, reliable transportation, and acceptable DMV driving record required.
- Ability to complete a Livescan background check, Employment Physical, TB screening, and CPR certification.
- Ability to lift up to 50 pounds.
- Comfortable with bending, sitting, standing, and navigating stairs.
