AL-Assistant Wellness Director - LPN is responsible for overseeing the day-to-day operations of a designated nursing unit in a healthcare, long-term care, or assisted living facility. The Assistant Wellness Director ensures that all residents receive high-quality care, supervises nursing staff, and manages clinical workflows. This role includes a combination of leadership, clinical oversight, and administrative responsibilities to ensure that the unit runs smoothly while maintaining compliance with regulatory requirements.
Unit Leadership and Supervision:

Oversee and manage nursing staff, including LPNs, RNs, and Certified Nursing Assistants (CNAs), ensuring that all care provided meets facility standards.

Conduct performance evaluations, provide feedback, and support staff development through coaching and mentorship.

Ensure that staffing levels are adequate for each shift by creating and managing staff schedules.

Lead daily shift handovers and team meetings, ensuring that all staff are informed of changes in residents’ conditions and care plans.
Patient Care Management:

Monitor the delivery of care within the unit to ensure residents' needs are met, including overseeing medication administration and treatments.

Collaborate with the interdisciplinary care team to develop, implement, and update individualized care plans for residents.

Conduct regular assessments of residents’ health conditions and make adjustments to care plans as needed.

Serve as a clinical resource to staff, providing guidance and support in handling complex resident cases.
Regulatory Compliance and Documentation:

Ensure that the unit complies with all federal, state, and facility regulations, including infection control protocols, safety measures, and patient privacy standards.

Maintain accurate and timely documentation in the electronic health record (EHR) system, ensuring compliance with documentation requirements.

Participate in facility audits, surveys, and inspections, implementing corrective actions as needed to address any deficiencies.

Conduct regular reviews of clinical documentation to ensure accuracy and completeness.
Quality Improvement and Resident Advocacy:

Implement quality assurance and improvement initiatives to enhance resident care and improve overall unit performance.

Address resident and family concerns promptly and professionally, ensuring their satisfaction with the care provided.

Act as an advocate for residents, ensuring that their rights are respected and that care is delivered with dignity and compassion.
Collaboration and Communication:

Collaborate with other department heads, including social services, dietary, and rehabilitation, to ensure comprehensive care for residents.

Maintain open and effective communication with residents' families, updating them on their loved ones' health status and care plans.

Participate in care conferences, interdisciplinary meetings, and care planning discussions to ensure coordinated care delivery.

Education: Graduate of an accredited LPN program.

Licensure: Active and unrestricted Licensed Practical Nurse (LPN) license.

Experience: Minimum of 2-3 years of nursing experience, preferably in a long-term care or healthcare setting, with at least 1 year in a supervisory or leadership role.
