Salary: $150,000 Annually
Aspire Home Healthcare has been putting patients first for 30 years. We're currently seeking a full-time Physical Therapist to provide in-home care to patients in Delaware, OH and nearby communities.

As a Physical Therapist (PT), you'll evaluate patients, develop and implement individualized treatment plans, and collaborate with our rehab team to deliver high-quality care in a home health setting.

Evaluate patient condition and functional level

Develop and implement personalized treatment plans

Supervise PTAs and HHAs per plan of care

Provide in-home therapy and adaptive equipment guidance

Maintain accurate and timely documentation

Educate patients and families on home exercise programs

Submit notes and visit documentation promptly

Adhere to HIPAA confidentiality standards

Active PT license in Ohio

1+ year of PT experience (preferred)

OASIS and WellSky/Kinser experience (strongly preferred)

Current TB test and CPR certification

Valid driver’s license and auto insurance

Ability to attend orientation in Dublin, OH
