The Role
We're looking for an experienced, compassionate, and confident Hospice Admissions RN who connects naturally with patients and families.
This is much more than completing paperwork and obtaining signatures.
You'll often meet families at a moment when they have difficult questions and important decisions to make. Your job is to listen, assess, educate, and help them understand what hospice can provide. When hospice is the right choice, you'll help make the transition into our care feel reassuring, organized, and seamless.
You'll also perform the clinical assessment necessary to determine hospice eligibility and ensure our care team has what they need to begin providing exceptional care from day one.
What You'll Do
- Complete hospice evaluations, admissions, and start-of-care visits in patients' homes and care facilities
- Perform comprehensive nursing assessments and evaluate patients for hospice eligibility
- Explain the hospice benefit, philosophy, services, and plan of care in a way patients and families can easily understand
- Help patients and families work through questions, concerns, and misconceptions about hospice
- Build trust quickly and provide calm, compassionate guidance during emotionally difficult situations
- Coordinate with physicians, referral partners, our Intake Team, and interdisciplinary care team
- Ensure accurate and timely admission documentation
- Create a seamless handoff so patients and families feel cared for—not passed off—once the admission is complete
- Represent AlēvCare with professionalism, warmth, and integrity in every interaction
What We're Looking For
Required:
- Current, unrestricted Texas RN license
- Minimum of 3 years of RN experience
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
- Ability to travel throughout our service area
- Ability to perform the physical requirements of a field-based nursing position
Strongly Preferred:
- Hospice nursing experience
- 2+ years of hospice admissions experience
- Experience discussing hospice eligibility and the Medicare Hospice Benefit with patients and families

Schedule
- Full-time
- Primarily daytime hours with flexibility based on patient and family needs
- Field-based position serving patients throughout the following counties and surrounding areas (Tarrant, Johnson, Parker, Hood)
