MDS RN (Registered Nurse) – Full-Time

Complete accurate and timely MDS assessments in accordance with CMS guidelines.

Coordinate the RAI process, CAAs, and care plans.

Collaborate with nursing, therapy, dietary, social services, and physicians.

Monitor Medicare and Medicaid documentation for compliance and reimbursement.

Identify significant changes in resident condition and ensure appropriate assessments are completed.

Participate in Quality Measures and Performance Improvement initiatives.

Maintain compliance with federal and state regulations.

Current Ohio RN license required.

MDS experience in a skilled nursing or long-term care setting required.

Strong knowledge of RAI process, Medicare, Medicaid, and CMS regulations.

Excellent assessment, documentation, organizational, and communication skills.

RAC-CT certification is a plus but not required.
