We are seeking a compassionate, organized, and personable Patient Care Coordinator to join our growing primary care practice. This is an in-office, phone-based position responsible for coordinating patient care, scheduling appointments, reviewing medication lists, and communicating with patients, providers, pharmacies, and specialists to ensure an exceptional patient experience.
The ideal candidate is friendly, professional, and confident communicating by phone. They are knowledgeable about medical terminology and common medications, able to multitask in a fast-paced environment, and passionate about helping patients. We are looking for someone who is dependable, organized, and detail-oriented, with a commitment to timely follow-up, clear communication, and providing a positive experience throughout each patient’s care journey.
Responsibilities
- Answer a high volume of inbound and outbound patient calls with professionalism and compassion.
- Coordinate patient care by scheduling appointments and follow-up visits.
- Review patient medication lists and communicate medication information accurately with patients and providers.
- Demonstrate knowledge of common medications and medical terminology while assisting patients and coordinating care.
- Communicate with physicians, pharmacies, and specialists.
- Document patient interactions accurately in the electronic medical record (EHR).
- Maintain confidential and up-to-date patient records.
- Ensure timely follow-up with patients regarding appointments, referrals, medication questions, provider recommendations, and other care needs.
- Monitor and complete assigned tasks promptly while maintaining a high level of accuracy and attention to detail.
- Assist patients with questions regarding appointments, medications, and treatment plans.
- Collaborate with providers and clinical staff to ensure seamless patient care.
- Perform additional administrative duties as assigned.
Qualifications
- Completion of a Medical Assistant program (degree or diploma) is required.
- Healthcare experience is preferred.
- Strong knowledge of medical terminology and common medications.
- Strong telephone communication and customer service skills.
- Excellent organizational skills with the ability to multitask.
- Exceptional attention to detail and the ability to prioritize tasks while meeting deadlines in a fast-paced environment.
- Experience with electronic medical records (EHR) preferred.
- Ability to work independently and as part of a team.
- High school diploma or GED required.
- Pharmacy Technician certification is a plus.
