Location: White River Junction, Vermont / Lebanon, New Hampshire area
Pay Rate: $65/hr $70/hr
Contract: 13 Weeks
Shift: Days
Guaranteed Hours: 36 hours/week
Experience: Minimum 2 years Home Health experience required

About the Role
We are seeking an experienced Registered Nurse Case Manager for a Home Health contract opportunity serving patients across Vermont and New Hampshire service areas. This role is ideal for a compassionate RN who enjoys building strong patient relationships, working independently, and coordinating care as part of an interdisciplinary team.
Responsibilities
Screen, evaluate, re-evaluate, and treat home health patients
Develop individualized care plans with physicians, patients, families, and caregivers
Coordinate patient care through interdisciplinary communication and case conferences
Assess clinical, functional, and emotional status throughout the episode of care
Communicate significant patient changes to physicians and care team members
Document evaluations, treatment goals, progress, and care activities in EPIC
Educate patients and caregivers on medications, diet, self-care, and health management
Requirements
Active Registered Nurse license in Vermont and New Hampshire, or unencumbered compact license required
Graduate of an accredited school of nursing required
Minimum 2 years of Home Health experience required
BLS/BCLS required
Strong case management, communication, and patient education skills
Ability to work independently in a home health setting
