Unity Care Group has a minor aged client seeking a new DSP to add to their team to help provide care for roughly 10-20 hrs/week. Most of this will be on the weekends. We are looking to quickly add someone to their team that can help them with activities of daily living.
This client has an amazing support team that is looking to add someone to pick up some hours. Guardian has some flexibility in what hours could be worked, and would prefer someone that has availability for 1st and 2nd shift and can work out shifts that makes sense.
Job duties can include but are not limited to:
- Providing support to the individuals served including healthcare, nutrition and program implementing.
- Assisting Individuals with daily living skills and the development of independent community skills.
- This may include meal preparation, cleaning, organizing, taking out into the community for activities.
- Participating in daily care of the individual.
- Assist with activities of daily living
- Accurately completing all on-site documentation; maintain current case notes and reports of Individuals progress and how it relates to their ISP.
- Coordinating and providing transportation for the individuals served.
- Ensuring cleanliness and upkeep of client home.
- Ensuring the safety and wellbeing of the client.
Requirements for this position include:
- High school diploma or GED
- CPR/FA Certificate
- Annual DSP Certificate
- Valid driver's license
- Less than 6 points on driving record
- Must be able to pass a background check
- Must be able to pass a drug test
- Must be able to pass a physical
- 1 Year Direct Service Professional (DSP) experience
