Responsibilities
- Lead and supervise the home health care team, including nurses, therapists, and support staff, ensuring high standards of patient care and team collaboration.
- Oversee patient assessments, develop individualized care plans, and monitor progress to optimize health outcomes.
- Ensure compliance with all state healthcare regulations, HIPAA privacy standards, and organizational policies related to medical documentation and patient safety.
- Coordinate with physicians, case managers, and other healthcare providers to facilitate seamless care transitions and discharge planning.
- Manage utilization review processes, including ICD-10 coding, DRG documentation, and medical record audits to support accurate billing and reimbursement.
- Oversee EMR (Electronic Medical Records) systems such as Wellsky
- Conduct staff training on clinical protocols such as IV infusion, catheter care, ventilator management, infection control practices, and basic life support procedures.
- Work on the field as needed.
Experience
- Proven nursing leadership experience with a strong background in home health or related settings such as hospital medicine or long-term care facilities.
- Extensive knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/ICD-10), CPT coding, and diagnostic evaluation techniques.
- Hands-on experience with EMR/EHR systems; proficiency in medical documentation and medical records management is essential.
- Demonstrated ability to manage teams effectively within regulated environments such as nursing homes or assisted living facilities.
- Experience working with geriatrics.
- Strong understanding of state healthcare regulations, long-term care regulations, Medicare/Medicaid policies (NCQA standards), and workers' compensation law where applicable.
- Skilled in patient assessment techniques including vital signs monitoring, physical examinations, specimen collection/processing, and discharge planning.
- Knowledge of infection control protocols, sterile processing techniques such as suturing or gastric lavage procedures is a plus; certification in basic life support (BLS) required.
