Overview:
We have a variety of PT/PTA openings available and are excited about matching you with a great opportunity! As a Heavenly Touch Home Health Care PT/PTA you will conduct an initial evaluation following physician referrals and develop appropriate follow up treatment plans. Maintain compliance with our agency’s policy and procedures and Medicare regulations. Notify the physician and/or other personnel of any change(s) in the patient’s condition and case conference clinicians providing care to ensure coordination of care and fulfill rehabilitation. As part of our team, you are required to attend staff meetings, as well as complete annual in-services hours.
Qualifications:
- One (1) year direct patient care experience in a clinical care setting, home health setting and/or hospice setting
- Good documentation skills required
- Ability to travel per job requirement
Requirements:
- High school graduate or equivalent
- State license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
- AHCA Background screening

Pay: $40.00 - $75.00 per hour
