- Provide direct nursing care to patients in the home setting in accordance with the plan of care
- Assist patients with activities of daily living (ADLs), including bathing, feeding, personal hygiene, and mobility
- Monitor and document vital signs, patient condition, and changes in status
- Administer medications and treatments as ordered by the physician and per state guidelines
- Provide tracheostomy and ventilator care (preferred)
- Monitor ventilator settings, alarms, and patient response (training provided if no prior experience)
- Perform suctioning, oxygen therapy, and airway management as needed
- Observe for signs of respiratory distress, infection, or other complications and report promptly
- Maintain accurate and timely clinical documentation
- Educate and support patients and families on basic care and safety
- Follow infection control, safety policies, and HIPAA regulations
- Communicate effectively with RNs, physicians, and the interdisciplinary care team
- Respond appropriately to emergency situations
