Overview
We are seeking a dedicated and compassionate Home Health office personal to oversee the delivery of high-quality care to patients in their homes. Responsibilities
- Supervise and coordinate home health services to ensure compliance with regulatory standards and best practices.
- Develop individualized care plans tailored to each patient's needs, collaborating with healthcare professionals and family members.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Conduct regular assessments of staff performance, providing training and support to enhance service delivery.
- Maintain accurate documentation of patient care activities and ensure confidentiality is upheld.
- Facilitate communication between patients, families, and healthcare providers to promote continuity of care.

Experience
- Experience Healthcare Administration, or a related field is preferred.
- Previous experience in home health management or a similar role is highly desirable.

- Proficient in using electronic health record (EHR) systems and other relevant software applications.
