Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care team. This role provides skilled nursing care to a diverse resident population, including individuals with Multiple Sclerosis (MS), orthopedic conditions, dementia, and memory care needs. The ideal candidate has LTC experience, strong clinical skills, and a compassionate approach to resident-centered care.
Responsibilities
- Monitor and record patient vital signs, symptoms, and overall condition
- Administer medications and treatments as prescribed by physicians
- Assist patients with daily living activities, including hygiene, mobility, and nutrition
- Maintain accurate and up-to-date patient records and documentation
- Collaborate with RNs, physicians, and interdisciplinary care teams to develop and implement care plans
- Educate patients and families on health management and post-care instructions
- Respond promptly to patient needs and escalate concerns to appropriate clinical staff
- Ensure compliance with facility policies, infection control standards, and regulatory requirements
Requirements
- Active, unrestricted LPN license in New York
- Minimum 1+ years of experience in a Long-Term Care (LTC) facility
- Current BLS certification
- Valid driver's license with address matching demographics
- Requested Time Off (RTO) must be submitted at time of application
- Availability for Every Other Weekend (EOW) shifts
