DIRECT SUPPORT PROFESSIONAL
DEFINITION: A qualified individual responsible for supporting individuals with disabilities in their day-to-day living needs; including habilitation, building community and family/friend supports, supervision, health and safety and providing choices to enhance their quality of life.
DUTIES AND RESPONSIBILITIES: (Essential job functions in bold)
Individual/Facility:

Provides direct support and supervision to individuals.

Responds to individuals' changing needs.

Maintains/Monitors the safety and cleanliness of home/apartment.

Participates in assigned assessments, individualized planning, and skill training and behavioral deceleration programs, including recording data.

Observes and reports all health and medical needs of the individuals to Supervisor and keeps parties informed of the individuals' well-being.

Assists individuals in the self-administration of medications and keeps documentation as assigned.
- Assists individuals in arranging outings, in making purchases, and providing opportunities for community integration and inclusion.

Assists individuals in menu planning, shopping, preparation of meals, and cleanup.
- Intervenes effectively in response to challenging behaviors exhibited by individuals served.

Document in Data Management System and reports suspected incidents of abuse, exploitation, and neglect to APS, the Director of Services and other appropriate agencies and individuals.

Compose Incident Report in Data Management System that affect the Health and Safety of individuals served.

Assist individuals with personal care as needed.

Implements training curricula, activity plans and materials which promote the personal and social adjustment, work performance and community integration of individuals

Implement activities promoting community integration, social and leisure interests, health and wellbeing of individuals served.

Accompanies and/or transports individuals to community events/outings, medical appointments and work.

Ensures that inside and outside areas of homes are orderly and well kept.
- Identifies need for adaptive equipment and follows up on implementation.

Conducts and documents fire drills and emergency procedures as necessary.

Performs other job-related duties as assigned.
General:

6. Attends all mandatory orientation and training activities, scheduled staff meetings and in-services/training as requested by Supervisor.
7. Establishes and maintains effective working relationships with individuals, co-workers, parents, agencies, and the public.
8. Maintain professional and appropriate boundaries.
9. Uses communication skills to participate effectively as a team member.
10. Uses equipment and supplies efficiently and informs Supervisor of shortages/needs.
11. Documents accurately work performed on Paycom and submit in a timely manner to Supervisor for approval.
12. Documents in Data Management Systems accurately and in a timely manner.
13. Safeguards and keeps confidential all Private and Personal Information regarding individuals with disabilities received in the course of their employment by the Corporation.
14. Practice safe work habits and follow all safety regulations and company procedures.
15. Maintain strict confidentiality.

High school diploma or equivalent and prior experience working with individuals with disabilities is preferred.
- No record of abuse/neglect or exploitation of others.

Must have or obtain a valid Montana driver's license, have an acceptable driving record, be insurable and present proof of private auto insurance in compliance with Montana State law within 1 month of hire.

Complete all required trainings within 30 days of hire.
