JOB SUMMARY: Bridging Access to Care is looking for a dedicated Women and Girls Health Educator with experience in community outreach, health education, HIV testing, and linkage to care services. Skilled in empowering women and girls, particularly those ages 25–34 at increased risk for HIV, STIs, HCV, and substance misuse, to make informed health decisions and access essential healthcare resources. Experienced in facilitating evidence-based interventions, including Parenting Journey and WILLOW (Women Involved in Life Learning from Other Women), focusing on HIV prevention, healthy relationships, safe-sex practices, self-awareness, and positive family dynamics. Adept at collaborating within multidisciplinary teams to provide individualized support, coordinate referrals, and connect clients to navigation and support services. Passionate about advancing sexual health education, HIV prevention, and community wellness initiatives throughout Central Brooklyn.
This role requires up to 75% of travel and street outreach. This role will require some evenings and
weekends.
EDUCATION AND QUALIFICATIONS:
- MINIMUM: High School Diploma/ GED with at least two (2) years of professional experience in the field of working with adult women
- Bachelor’s degree in health education or related field and one (1) year of experience
- MINIMUM: For High School Diploma/GED Candidates: Two (2) years’ experience working in substance use group facilitation with the target population
- Experience with conducting, client recruitment, and networking, which includes prioritizing the population of focus.
- Up to 75% of travel is required.
LICENSURE:
- A NYS Driver’s License with at least a two (2) year clean driving record, preferred. A clean driving history is defined as no moving violations or suspensions.
DUTIES AND RESPONSIBILITIES:
Program Delivery & Interventions
- Deliver evidence-based practice (EBP) interventions to fidelity, conducting all phases as outlined in the following curriculum: Deliver high-impact, evidence-based interventions, including Parenting Journey and WILLOW in group settings.
- Conduct all phases of the program curriculum as outlined.
- Ensure the program services and deliverables meet contract requirements.
Partnerships & Outreach:
- Create linkages and partnerships with other providers to recruit the target population.
- Attend outreach events, health fairs, and tabling opportunities to recruit eligible participants for program enrollment.
- Attend monthly outreach meetings and participate in evening or weekend events as necessary.
- Create engaging content for social media platforms to reach the target audience.
- Post at least one piece of content per month and aim for a minimum engagement of five people per post.
Administrative & Reporting:
- Maintain and update Outlook and eCR calendars regularly.
- Ensure all deliverables are met as stipulated in the program contract.
- Maintain updated and accurate individual records for all clients via eCR.
- Complete monthly reports and close client charts according to BAC and department standards.
Meetings & Collaboration
- Attend the monthly Outreach and Partnership meeting.
- Participate in Continuous Quality Improvement (CQI) projects.
- Attend agency committee meetings, supervision, team, and department meetings.
- Collaborate effectively with team members and complete additional tasks assigned by the supervisor.
Professional Development & Compliance
- Attend relevant training and conferences about the program and target audience.
- Join community committees, task forces, or networks related to the program and/or target audience.
- Adherent to the business casual dress code.
- Perform other duties as assigned based on the needs of the program.

KNOWLEDGE, SKILLS, AND ABILITIES:
- Ability to work with multiple targeted populations such as adult women, HIV+, high-risk/at-risk individuals.
- Ability to conduct HIV Testing and Hepatitis C testing, a plus
- Previous experience delivering evidence-based interventions and/or group facilitation
- Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodations.
- Must be willing to work weekends, nights, and long/flexible hours as required by the position.
- Outstanding writing and communication skills, including Twitter/Facebook and other multimedia outlets.
- A NYS Driver’s License in good standing, a plus
- Strong outreach Skills, including creating partnerships,
- Familiar with using EHR systems, a plus.
- Effective communication, including writing, speaking, active listening, and presenting.
- Up to 75% of travel is required.
