What You’ll Do:
- Monthly Visits: Conduct routine home visits, document ADLs, and submit complete clinical notes within 24 hours.
- Care Planning: Use your expertise to develop, update, and individualize member Plans of Care.
- Assessments & Compliance: Handle semi-annual health reports, annual MDS reviews (including psychosocial/fall risks), and coordinate with PCPs.
- MDT Collaboration: Call into monthly multidisciplinary team meetings to present quick case summaries.
What We’re Looking For:
- Active LPN License in Massachusetts (Required)
- Home health or community-based nursing experience (Highly preferred—your background means you can hit the ground running!)
- Reliable transportation for local in-home visits
- Sharp documentation skills and strong time management
- Experience with EMR networks is a huge plus!
