Rose View Nursing and Rehabilitation Center is seeking an experienced and detail-oriented RNAC (Registered Nurse Assessment Coordinator) to join our clinical leadership team. The RNAC plays a vital role in coordinating the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, supporting care planning, and optimizing reimbursement while maintaining compliance with all federal and state regulations.
This position is ideal for a compassionate Registered Nurse with strong clinical assessment skills, knowledge of skilled nursing reimbursement systems, and a commitment to quality resident care.
Coordinate and complete MDS assessments in accordance with RAI guidelines and regulatory deadlines
Review clinical documentation to ensure accuracy and support for MDS coding and reimbursement
Collaborate with the interdisciplinary team to develop, update, and maintain individualized resident care plans
Monitor MDS scheduling and ensure timely transmission of assessments
Assist with Medicare and Medicaid reimbursement processes, including case mix management
Ensure compliance with CMS regulations, state guidelines, and facility policies
Communicate effectively with nursing leadership regarding resident status changes and assessment findings
Participate in quality assurance and performance improvement initiatives
Support survey readiness efforts and maintain regulatory compliance standards
Provide guidance and education to clinical staff regarding documentation and MDS processes
Active Registered Nurse (RN) license in Pennsylvania
Previous RNAC/MDS Coordinator experience in a skilled nursing facility preferred
Strong knowledge of RAI guidelines, care planning, Medicare/Medicaid reimbursement, and case mix systems
Excellent assessment, organizational, and critical thinking skills
Ability to work collaboratively within an interdisciplinary team environment
Experience with electronic medical record (EMR) systems preferred
Strong communication and time management skills
