Responsibilities:
- Provide high-quality patient care to residents with diverse needs, including those requiring short-term therapy and long-term care
- Develop and implement individualized care plans in collaboration with interdisciplinary team members
- Administer medication, treatments, and other therapies as prescribed by physicians
- Monitor and report patient progress, identifying areas for improvement and implementing modifications as needed
- Foster positive relationships with residents, families, and staff to ensure a warm and welcoming environment
- Participate in quality improvement initiatives to enhance the overall quality of care
