American Traveler is seeking an experienced RN Case Manager for a contract position at a rehabilitation hospital in Cambridge, MA, requiring an active MA RN license and Epic experience.

Day shift position, 8:00 AM – 4:30 PM

Case Management unit within a rehabilitation hospital setting in Cambridge, MA

Responsibilities span both inpatient coordination and referrals for post-discharge community services

Uses Epic for charting

UKG/Kronos used for timekeeping

Active MA RN license required — no pending or conditional licenses accepted

Minimum 2 years of case management experience required

BLS certification required — AHA only; Red Cross and other providers are not accepted for this facility

Epic EMR experience required

2 professional references required for consideration

Flu vaccine required; no religious or medical exemptions are accepted

Conducts client assessments and develops individualized care plans in collaboration with patients, families, and service providers

Modifies treatment plans based on patient responses and changing conditions
-
Communicates with third-party payers to obtain authorizations for reimbursement of services

Coordinates discharge planning and prepares patients and families for transitions to community-based care

Maintains accurate documentation and detailed case records

Collaborates with the medical team on complex or challenging cases
