We are seeking a licensed Physical Therapist (PT) to join our interdisciplinary team. Care and compassion are at the heart of what you'll do. You'll provide physician-directed, home-based therapy, collaborate with patients and their families, and help guide recovery while delivering quality, personalized care wherever patients call home.
- Agency: Alternate Solutions Home Health Columbus
- Schedule: M-F 8am-5pm
- On Call: One weekend every 2 months.
- Salary: From $91,500.00 per year
- Territory: Circleville, Derby, Harrisburg, London, New Holland, Orient, Washington Court House, Williamsport areas

You will...
- Complete evaluations, admissions, and OASIS assessments
- Develop and modify individualized plans of care
- Provide skilled physical therapy and patient/caregiver education
- Monitor patient progress and communicate with physicians and care teams
- Supervise PTAs and coordinate patient care as needed
- Complete accurate, timely documentation in compliance with Medicare and agency guidelines
- Promote continuity of care and help reduce hospital readmissions

- Licensure: Current, unrestricted Physical Therapist (PT) license in the state of employment.
- Experience: Minimum of 1 year of experience as a PT in an acute care setting is required. Prior home health or community-based care experience is preferred.
- Skills: Demonstrated proficiency in physical therapy best practices, independent clinical judgment, acute problem-solving, and time management.
- Other Requirements: Valid driver's license, reliable transportation, and current auto insurance.
