We are seeking a compassionate, organized, and motivated Family Support Coordinator to support infants, toddlers, and their families through Louisiana's EarlySteps Program. Position Summary
The Family Support Coordinator serves as the primary point of contact for families enrolled in EarlySteps. This role focuses on coordinating early intervention services, empowering families through education and support, and connecting them with community resources to help their children succeed.
What You'll Do
- Conduct quarterly in-home visits with families (virtual visits when appropriate).
- Coordinate and facilitate evaluations, assessments, and Individualized Family Service Plan (IFSP) meetings.
- Develop and maintain strong, supportive relationships with families.
- Connect families with community resources and support programs.
- Coordinate services with therapists, physicians, childcare providers, and other community partners.
- Complete accurate documentation and maintain compliance with EarlySteps and agency requirements.
- Manage an approved caseload while meeting required deadlines.
- Travel within assigned parishes to provide family-centered services.

Preferred Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Education, Nursing, or a related field.
- Experience working with children, families, case management, social services, healthcare, or early intervention is preferred.
- Knowledge of Louisiana EarlySteps or IDEA Part C is a plus but not required.
