The Role
We're looking for a Care Manager to join our growing team and walk alongside clients and families as needs change over time. You'll assess situations, develop clear care plans, and coordinate medical, social, and community supports, often during stressful or uncertain moments.
This is a part-time, hourly W-2 role, paid for all client-related work including calls, emails, care coordination, documentation, travel, client visits, and appointment attendance. Typical schedules are 15 to 30 hours per week, Monday through Friday, daytime hours, with generally consistent hours week to week. There is occasional after-hours and weekend work to attend to client emergencies, billed at overtime rates.
New Care Managers are not handed a caseload cold. You'll shadow experienced care managers on real client visits before taking on your own, so you start this role with real support and context behind you.

Key Responsibilities
- Conduct comprehensive assessments (medical, functional, psychosocial)
- Develop individualized Care Plans
- Build trusted relationships with clients and families
- Coordinate care across providers and services
- Communicate clearly and consistently with families and responsible parties
- Attend key medical appointments as needed
- Support medication oversight and refill coordination
- Document care clearly and professionally
Qualifications
- Active license as a Registered Nurse, Occupational Therapist, Physical Therapist, or Social Worker
- Calm, compassionate presence and strong clinical judgment
- Clear communication and follow-through
- Comfort working independently
- Prior care management or geriatrics experience preferred
- CCM or CMC certification a plus
