Salary Range: $72,000-$80,000 per year
VISIONS Citywide Caregiver Program, funded by NYC Aging, provides comprehensive support to unpaid caregivers caring for an adult age 60+, an adult of any age with dementia or Alzheimer’s, an adult age 18–59 with a disability (when the caregiver is age 55+), or children under 18 cared for by older family members. VISIONS serves caregivers of individuals with vision loss, hearing loss, dementia, and other disabilities across all five boroughs, in multiple languages and accessible formats.
The Care Manager is a key member of the Caregiver Program team, supporting the goals and service delivery requirements of the program. The Care Manager is responsible for providing direct case management services, including screening, intake, assessment, care planning, service coordination, and ongoing monitoring for caregivers and care receivers. The Care Manager assesses client needs and eligibility for caregiver services and manages all aspects of the case in accordance with NYC Aging program standards, including required timelines for service delivery, documentation, follow-up, and reassessment.
- Conduct preliminary screening of applicants and determine program eligibility; initiate contact with all potential clients within 24 hours of referral or inquiry.
- Provide accurate and appropriate information regarding caregiver services, program eligibility, and available supports.
- Complete intake and provide individualized direct care management services to caregivers, including information and referral, case assistance (including benefits and entitlements), supportive counseling, and ongoing support.
- Conduct comprehensive in-home or virtual assessments of caregivers and care receivers to evaluate social, emotional, and environmental needs and determine eligibility for services, including respite and supplemental supports, within 10 business days of completing intake.
- Develop, implement, and manage individualized caregiver support plans in collaboration with the caregiver; monitor progress and ensure appropriate service utilization through to case closure.
- Coordinate and authorize services, including respite and supplemental services; submit required documentation for approval and coordinate with vendors to ensure timely service delivery.
- Make referrals to internal and external providers and conduct follow-up within 10 business days of referral to confirm linkage, service initiation, and caregiver satisfaction.
- Maintain ongoing case monitoring through regular check-ins and reassess caregiver needs; complete formal reassessments at minimum every 6 months, or more frequently as needed based on changes in caregiver circumstances.
- Develop and facilitate caregiver support groups and assist with trainings and workshops, as assigned.
- Work collaboratively with VISIONS Center on Blindness staff and other internal programs to coordinate group and overnight respite services.
- Maintain accurate, complete, and timely documentation of all client interactions, assessments, services, and case notes in the NYC Aging database system (VIVE), ensuring all documentation is entered within 5–10 business days of service delivery in accordance with program standards.
- Track and report all units of service, ensuring timely submission and alignment with program requirements and performance targets.
- Participate actively in supervision, case conferences, and staff meetings to support quality care and compliance with program standards.
- Supervise social work interns, as assigned, including obtaining SIFI certification within one year of employment.
- Arrange for interpretation and translation services, as needed, to ensure culturally and linguistically appropriate service delivery.
- Complete all work assignments thoroughly, accurately, and within established organizational and contractual timeframes.
- Perform other duties as assigned to support program operations and service delivery in the agency.
Master’s degree in social work (MSW) or a related field (e.g., social services, public administration, public health) is preferred. Bachelor’s degree with relevant experience may be considered.
- Minimum of 3 years of experience working with older adults, caregivers, and/or individuals with disabilities in a social service or healthcare setting.
- Demonstrated ability to provide case management services, including intake, assessment, care planning, service coordination, and follow-up.
- Strong interpersonal and communication skills, with the ability to engage caregivers in a respectful, culturally responsive, and person-centered manner.
- Fluency in English required; proficiency in Spanish, Chinese, or other languages commonly spoken in New York City is preferred.
- Experience working in NYC Aging or similar government-funded programs preferred.
- Ability to travel independently throughout all five boroughs of New York City to conduct home visits, outreach, and service coordination.
- Strong organizational and time management skills, with the ability to manage multiple cases and meet required timelines and documentation standards.
- Ability to work effectively with individuals from diverse cultural, linguistic, and socioeconomic backgrounds.
- Proficiency in computer applications, including data entry systems and electronic case management systems; experience with VIVE or similar systems preferred.
- Strong attention to detail and ability to manage multiple tasks with accuracy in a deadline-driven environment.
- Ability to work both independently and collaboratively as part of a multidisciplinary team.
- Strong interpersonal skills with the ability to interact effectively with clients, staff, vendors, and community partners.
- Demonstrated organizational and problem-solving skills, with the ability to prioritize workload and meet required timelines.
- Excellent written and verbal communication skills, including the ability to document clearly and professionally.
- Proficiency in computer applications, including Microsoft Word, Excel, email, internet-based systems, and data entry.
- Ability to learn and effectively utilize electronic case management and participant tracking systems (e.g., VIVE).
- Understanding of and commitment to the mission and philosophy of a nonprofit social service organization, including sensitivity to the needs of caregivers and individuals with disabilities.
