Member Duties : Help independent living residents at St. John's United improve community connections so they can lead healthy and productive lives! The overall goal of this project is to develop a Resource Directory and accessible network of supportive services for low-income older adults who reside at or receive services from St. John’s United. The VISTA will: complete a Social Determinants of Health (SDOH) survey of residents of St. John’s low income housing project, Chapel Court, to identify community need; create a sustainable Volunteer Plan with a calendar to help increase the mentor base to strengthen and expand resident access to Community Resources; research available providers and services that serve the identified needs to create a Resource Guide; create a Marketing Plan to inform the necessary target audience about the Resource Guide; and build relationships with community resource agencies and health providers to improve services available to residents.
