Position Overview
We are seeking an experienced Clinical Pharmacist with Long-Term Care (LTC), hospital, or acute care experience to provide high-quality pharmaceutical care. This role involves medication management, patient counseling, sterile compounding, clinical collaboration, and ensuring safe and effective medication use.
Key Responsibilities
- Review, verify, and dispense medications accurately
- Collaborate with physicians and healthcare teams on treatment plans
- Conduct medication therapy reviews and patient assessments
- Prepare sterile and non-sterile compounded medications as needed
- Manage IV therapies, immunizations, and medication safety initiatives
- Maintain accurate documentation using EMR/EHR systems
- Oversee pharmacy operations, inventory, and technician workflow
- Educate patients on medications, side effects, and treatment plans
Qualifications
- Active Pharmacist License required
- Experience in LTC, hospital, acute care, or critical care settings preferred
- Strong knowledge of sterile compounding, IV therapy, and medication management
- Familiarity with EMR/EHR systems and pharmacy software
- Excellent communication and patient care skills
