Job Summary
We are seeking a dynamic and compassionate Visiting Nurse Practitioner to join our healthcare team, delivering high-quality patient care to patients in short-term and long-term post-acute settings and Assisted Living communities. In this vital role, you will provide comprehensive clinical services, including assessments, diagnoses, treatment plans, and follow-up care in facilities in the Northern Seattle and Snohomish areas, 3-5 days per week, with a daily expectation of 15 visits. Your expertise will help improve health outcomes, promote independence, and enhance quality of life for individuals of all backgrounds. Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine health status and needs.
- Develop and implement individualized care plans aligned with best practices and patient goals, incorporating chronic disease management, acute care, geriatrics, pediatrics, and behavioral health as appropriate.
- Prescribe, monitor, and adjust medications and treatments following strict protocols.
- Utilize electronic health record (EHR) systems like Epic or Gehrimedfor accurate documentation of patient encounters, treatment plans, and medical records while maintaining HIPAA compliance.
- Collaborate with multidisciplinary teams on discharge planning, case management, utilization review, and coordination of behavioral health or hospice care services.
Skills
- Proven experience working with diverse populations including seniors in assisted living or nursing home environments; experience with dementia care or memory care is a plus.
- Strong knowledge of medical terminology, physiology, anatomy, diagnostic evaluation techniques such as sonography or gastric lavage.
- Familiarity with EMR/EHR systems such as Epic or Gehrimed; proficiency in CPT coding (Current Procedural Terminology) and ICD coding (International Classification of Diseases).
- Excellent patient assessment skills with the ability to manage acute pain cases and infectious disease care confidently.
- Experience in chronic condition management.
- Capable of working independently in facility settings while adhering to infection control standards and OSHA regulations.
