Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility
- Monitor and observe patient conditions, including vital signs and overall health status
- Administer medications and perform catheter care following medical instructions and documentation protocols
- Support patients with memory care needs such as Alzheimer's or dementia by implementing personalized care plans
- Operate Electronic Health Record (EHR) systems to accurately document patient information and medical records
- Utilize patient transfer methods and equipment such as Hoyer lifts to ensure safe mobility assistance
- Provide companionship and emotional support while fostering effective communication with clients and their families
Requirements
- Proven experience working as a Home Health Aide or in assisted living, nursing home, or hospice settings
- Knowledge of medical terminology, medication administration, and home health care regulations
- Certification in First Aid and Basic Life Support (BLS), including CPR training
- Ability to perform heavy lifting and physical tasks safely using proper patient transfer methods and equipment like Hoyer lifts
- Familiarity with EMR (Electronic Medical Records) systems and medical documentation standards
- Experience working with individuals with disabilities or developmental disabilities preferred
- Strong understanding of HIPAA regulations to maintain patient confidentiality at all times
- Excellent communication skills for effective client interaction and team collaboration
- Valid driver’s license and reliable transportation are often preferred for home visits
