Key Responsibilities
- Patient Assessment: Conduct thorough evaluations of patients’ health, social, and care needs
- Care Coordination: Facilitate seamless communication between healthcare providers, patients, and families
- Care Planning: Develop and implement individualized, person-centered care plans
- Resource Management: Connect patients with community-based services and support programs
- Monitoring & Follow-Up: Track patient progress, adjust care plans as needed, and conduct follow-up visits or calls
- Education & Support: Educate patients and families about conditions, treatment plans, and resources
- Data Management: Maintain accurate documentation in compliance with regulatory standards
- Collaboration: Work closely with healthcare teams to ensure coordinated and effective care
What We’re Looking For
- Minimum 2 years of care management experience
- CHW (Community Health Worker) training/certification preferred
- Bilingual (English/Spanish) required
- Strong understanding of healthcare systems and community resources
- Excellent communication, interpersonal, and organizational skills
- Ability to work independently while collaborating effectively with a team
- Comfortable using electronic health records and care management software
- Willingness and ability to travel within Fremont/Alameda County
