Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, recording vital signs such as blood pressure, pulse, and temperature accurately
- Support medication administration following prescribed guidelines while maintaining strict adherence to HIPAA regulations and medical documentation standards
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques
- Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient information securely and efficiently
- Use assistive devices such as Hoyer lifts or catheters to ensure safe patient transfers and hygiene management
- Prepare nutritious meals aligned with dietary needs and assist with light housekeeping tasks like cleaning and laundry to maintain a safe environment
- Collaborate with nursing staff and healthcare professionals to coordinate patient care plans in home or community settings
- Support patients during hospice care by providing comfort measures and emotional support in accordance with care plans
Requirements
- Proven experience in home health care, assisted living, nursing homes, or long-term care environments
- Knowledge of medical terminology, medication administration protocols, and vital signs monitoring
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus
- Ability to work independently while following detailed care plans and medical instructions
- Experience working with individuals with developmental disabilities or disabilities is highly desirable
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Compassionate attitude with excellent patient observation skills and strong communication abilities
- Valid driver’s license and reliable transportation preferred for community-based visits
