Neighborhood Planner (Activities Assistant)
Location: SMP Health St. Raphael, Valley City, ND
Job Type: Full-Time, Monday-Friday, 8:00am-5:00pm
Overview:
Join our team to help create engaging activities that enhance the physical, social, and emotional well-being of our residents. The Neighborhood Planner works closely with the Activities Coordinator to plan, organize, and lead programs that make a difference every day.
Key Responsibilities:
- Develop and post monthly activity calendars.
- Assist residents in attending and participating in activities.
- Conduct one-on-one visits and in-room programs.
- Document attendance and progress notes accurately.
- Coordinate offsite trips and collaborate with other departments.
- Support Resident Council meetings and guide volunteers.
Qualifications:
- Valid CNA license or willingness to obtain.
- Strong communication, organization, and leadership skills.
- Ability to work flexible hours, including weekends/holidays.
- Basic computer skills and a positive, team-oriented attitude.
Physical Requirements:
Ability to move throughout the day, assist in emergencies, and perform common physical tasks (bending, lifting, etc.).

Pay: $18.60 - $23.25 per hour
