Position Overview:
We are currently seeking a Weekend On-Call Registered Nurse to support our patients and families on Saturdays and Sundays from 8:00am to 9:00pm. This role involves a mix of phone triage and in-home visits, addressing urgent needs, providing comfort, and ensuring continuity of care through strong interdisciplinary collaboration.
What You’ll Do:
- Provide compassionate nursing care in response to after-hours calls and urgent needs
- Coordinate care across the interdisciplinary team and ensure proper follow-up
- Perform skilled assessments, update plans of care, and communicate changes to the team
- Support families with education, medications, and emotional guidance
- Document timely and thoroughly in EMR
- Ensure all equipment, supplies, and services are in place as needed
What We’re Looking For:
- Colorado RN license (required)
- Prior hospice experience strongly preferred (or at least 1 year of recent med/surg or related experience)
- Excellent clinical judgment, documentation, and triage skills
- Ability to work independently, prioritize, and remain calm under pressure
- Reliable transportation and current auto insurance
- CPR certification
