Job Summary
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team in Phoenix, Arizona, for a 25-week contract assignment. The ideal candidate will have strong clinical skills and experience providing physical therapy services to patients recovering from injury, illness, surgery, or other conditions affecting mobility and functional independence.
The Physical Therapist will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, monitor progress, and collaborate with physicians and the interdisciplinary rehabilitation team to help patients achieve their functional goals.
Duties
- Perform comprehensive physical therapy evaluations and functional assessments.
- Develop individualized treatment plans based on each patient's diagnosis, condition, and functional goals.
- Provide therapeutic exercises, strengthening, stretching, balance training, gait training, and neuromuscular re-education.
- Assist patients with improving mobility, strength, coordination, endurance, balance, and functional independence.
- Provide transfer and mobility training as appropriate.
- Utilize appropriate therapeutic modalities and rehabilitation techniques based on patient needs.
- Monitor patient response to treatment and modify plans of care as clinically indicated.
- Establish measurable short-term and long-term rehabilitation goals.
- Educate patients and families regarding exercises, mobility, safety, home programs, and strategies for continued recovery.
- Communicate patient progress and clinical concerns to physicians and other members of the healthcare team.
- Participate in interdisciplinary care planning and rehabilitation meetings.
- Complete accurate and timely evaluations, daily treatment notes, progress reports, and discharge documentation.
- Assist with discharge planning and provide recommendations for continued therapy or follow-up care.
- Maintain compliance with facility policies, professional standards, and applicable state and federal regulations.
- Maintain a safe treatment environment for patients, families, and staff.
- Other duties as assigned.
Requirements
- Current and active Physical Therapist license in Arizona.
- Graduate of an accredited Physical Therapy program.
- Current BLS certification.
- Previous experience in rehabilitation, inpatient rehabilitation, acute care, skilled nursing, outpatient, or hospital-based physical therapy preferred.
- Strong knowledge of physical therapy assessment and treatment techniques.
- Excellent communication, organization, and documentation skills.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Ability to safely perform the physical requirements of the position, including assisting with patient mobility and transfers.
- Must demonstrate professionalism, compassion, and strong patient-care skills.
Preferred Qualifications
- Previous travel or contract physical therapy experience.
- Experience treating adult and geriatric rehabilitation patients.
- Experience with post-surgical, orthopedic, neurological, or medically complex patients.
- Familiarity with electronic medical records and rehabilitation documentation systems.
