Amada Senior Care is seeking an experienced Lead Care Coordinator / Client Care Manager to help lead client care services, support caregivers, conduct client assessments, coordinate services, and ensure exceptional care delivery throughout our service area.
What You'll Do:
Client Care & Assessments
- Conduct new client intakes and in-home assessments
- Develop, implement, and update individualized care plans
- Monitor client needs and changes in condition
- Maintain accurate client records and documentation
- Conduct supervisory and quality assurance visits
Client & Family Relations
- Build strong relationships with clients and families
- Serve as a primary contact for client concerns
- Resolve service issues professionally and compassionately
- Promote outstanding customer service and client satisfaction
Caregiver Leadership
- Support, coach, and supervise caregivers
- Conduct caregiver supervisory visits
- Communicate care plan updates and client expectations
- Promote caregiver engagement and retention
Scheduling & Service Coordination
- Collaborate closely with the scheduling team
- Assist with staffing challenges, call-offs, and schedule changes
- Help ensure appropriate caregiver-to-client matching
- Promote continuity of care and consistent service delivery
Quality Assurance & Compliance
- Monitor service quality and client satisfaction
- Conduct quality assurance visits
- Assist with regulatory compliance activities
- Identify and resolve care-related concerns

Qualifications
Required
- Minimum 2 years of experience in Home Care, Home Health, Hospice, Assisted Living, Skilled Nursing, Long-Term Care, or Senior Living
- Experience conducting client assessments, care coordination, caregiver supervision, scheduling, or client services
- Strong knowledge of ADLs, dementia care, and senior care needs
- Excellent communication and customer service skills
- Strong organizational and problem-solving abilities
- Valid driver's license and reliable transportation
Preferred
- Experience conducting in-home assessments and care planning
- Experience supervising caregivers or coordinating care teams
- Experience working with scheduling operations
- Experience using home care software such as AxisCare, WellSky, ClearCare, AlayaCare, CareSmartz360, MatrixCare, or similar systems
