Duties
- Provide strategic leadership for all clinical operations within the hospice program, ensuring high-quality patient care aligned with regulatory standards.
- Supervise nursing staff, including nurse managers and direct care nurses, promoting professional development and maintaining staffing efficiency through shift management.
- Oversee the development, implementation, and review of individualized care plans for residents, incorporating medical terminology, physiology, anatomy knowledge, and hospice best practices.
- Ensure compliance with HIPAA regulations and maintain the confidentiality of medical records and documentation review processes using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems.
- Monitor clinical documentation accuracy, review care plans regularly, and ensure proper coding for Medicare reimbursement.
- Lead interdisciplinary team meetings to coordinate resident care, including assessments related to MDS (Minimum Data Set) processes for skilled nursing facilities or assisted living communities.
- Manage staff scheduling across shifts, ensuring adequate coverage for home health visits, acute care needs, and resident support services.
- Foster a culture of leadership by supervising staff performance, conducting evaluations, and supporting ongoing education in areas such as medical terminology and nursing administration.
- Collaborate with physicians, social workers, and other healthcare providers to optimize resident outcomes while adhering to state and federal regulations governing hospice care.
Experience
- Proven management experience in nursing administration within assisted living facilities, nursing homes, home health agencies, or hospice programs.
- Demonstrated supervising experience with multidisciplinary teams in fast-paced healthcare environments.
- Extensive knowledge of hospice care standards, Medicare regulations, and documentation requirements related to end-of-life services.
- Strong background in nursing practice with familiarity in acute care settings or home health environments.
- Experience working with EMR/EHR systems for accurate medical records management and documentation review.
- Familiarity with patient's care plans development, HIPAA compliance, and medical terminology essential for effective leadership.
