Active Life Health Care Management LLC is a licensed Adult Day Health Care (ADHC) provider committed to enhancing the health, independence, and quality of life of adults and seniors in a supportive community setting. Our multidisciplinary team delivers skilled nursing, social, and therapeutic services tailored to each participant’s care plan.
Position Summary
The Registered Nurse (RN) is responsible for the clinical oversight, coordination, and delivery of nursing services to participants enrolled in our ADHC program. The RN ensures that all care is provided in accordance with physician orders, participant care plans, and state regulations (105 CMR 158.000).
Key Responsibilities
- Conduct participant health assessments, medication reviews, and vital sign monitoring.
- Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
- Administer medications and treatments as prescribed; document all nursing interventions accurately.
- Communicate with participants, families, caregivers, and physicians to support care continuity.
- Maintain compliance with Massachusetts Department of Public Health (DPH) and MassHealth ADH regulations.
- Participate in quality assurance activities and infection control monitoring.
- Support staff training and ensure that direct care personnel follow proper health and safety procedures.
- Respond to medical emergencies promptly and appropriately.
Qualifications
- Current Massachusetts RN license in good standing (required).
- Strong assessment, documentation, and communication skills.
- CPR certification (required).
- Knowledge of DPH, MassHealth, and ADH program standards preferred.
- Compassionate, dependable, and able to work effectively as part of a multidisciplinary team.
Job Type: Full-time
Pay: $40.00 - $48.00 per hour
