Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and review of medical history
- Develop and implement individualized care plans aligned with physician orders and patient needs.
- Educate patients and their families on disease management, medication use, pain management strategies, and health coaching to promote self-care and adherence to treatment plans.
- Coordinate with interdisciplinary teams for discharge planning, case management, and long-term care arrangements including assisted living or hospice & palliative medicine services.
- Monitor patient progress through regular evaluations of vital signs, laboratory results (including ICD coding), and physiological responses; adjust care accordingly.
- Ensure compliance with HIPAA regulations and maintain accurate medical documentation while managing complex cases involving trauma medicine or infectious disease care.
Experience
- Proven experience as a Registered Nurse in diverse settings.
- Prior home & community care experience preferred, especially working with individuals with disabilities or in geriatrics including memory care or dementia care environments.
- Knowledge of infection control protocols, aseptic techniques, medication management and more.
