The Occupational Therapist (OT) at Chestnut Park Rehabilitation and Nursing Center is responsible for evaluating, planning, and providing skilled occupational therapy services to short-term rehabilitation and long-term care residents. The OT works collaboratively with the interdisciplinary team to promote functional independence, enhance safety, and improve residents’ overall quality of life while ensuring compliance with all federal, state, and facility regulations.

Conduct comprehensive occupational therapy evaluations upon admission and as clinically indicated
Develop and implement individualized treatment plans in accordance with physician orders and resident goals
Provide skilled OT services focused on ADLs, fine motor skills, cognitive retraining, upper extremity function, positioning, splinting, and adaptive equipment training
Address functional mobility and environmental safety within resident rooms and common areas
Recommend and train residents and staff in the use of adaptive devices and compensatory techniques
Participate in interdisciplinary care plan meetings and communicate resident progress to team members
Complete accurate, timely documentation in compliance with Medicare Part A, Part B, PDPM, and New York State Department of Health regulations
Monitor treatment effectiveness and adjust plans of care as appropriate
Supervise and provide clinical oversight to Certified Occupational Therapy Assistants (COTAs) as assigned
Educate families and caregivers regarding home safety and discharge planning
Maintain infection control standards and follow all facility policies and procedures
Graduate of an accredited Occupational Therapy program
Current, active New York State Occupational Therapist license
Current CPR certification
Experience in skilled nursing, geriatric rehabilitation, or long-term care preferred
Knowledge of Medicare regulations and PDPM strongly preferred
Strong clinical assessment and treatment planning skills
Knowledge of age-related diagnoses including CVA, orthopedic conditions, cardiac conditions, dementia, and generalized deconditioning
Ability to collaborate effectively within an interdisciplinary team
Excellent documentation and time management skills
Strong communication skills with residents, families, and staff
Commitment to resident-centered care and quality outcomes
Ability to lift, transfer, and assist residents as required
Frequent standing, walking, bending, reaching, and demonstration of therapeutic exercises
Ability to respond quickly in emergency situations
