As a member of the Hospice of Dubuque Interdisciplinary Team, the Hospice Nurse Case Manager. Our new 4-day workweek schedule provides for greater work/life balance:
- assesses and responds to the needs of patients and families in a variety of settings and with varied lifestyles,
- communicates effectively with patients and families, fellow team members, and other healthcare organizations,
- participates in the development of the Interdisciplinary Team’s plan of care, and provides direct nursing care and client education within that plan of care,
- documents assessments and nursing care provided,
- participates in a nursing weeknight and weekend on-call rotation schedule that ensures nursing availability 24/7.
The Hospice Nurse Case Manager must:
- Possess compassion, sensitivity, and dedication, as well as strong problem-solving skills and he ability to work autonomously.
- Be physically able to load and unload medical equipment weighing up to 45 pounds, visit patients in their place of residence, and provide care to patients in low beds,
- have a valid driver’s license and acceptable driving record, reliable vehicle, adequate car insurance, and be able to drive in all types of weather,
- meet regulatory requirements and applicable laws and abide by Hospice of Dubuque’s policies and procedures, including the Standards of Conduct and Code of Ethics.
