Care nursing home patients with clear ownership and a treat-in-place approach.
- Provide primary care to a steady patient panel
- Lead on-site care planning and treat patients in place
- Assess patients and build clear care plans
- Manage chronic conditions, medications, and prior authorizations
- Close HEDIS (quality measures) and STARs care gaps
- Coordinate care with families and clinical teams
- Active Nurse Practitioner license in Ohio
- Current national NP certification
- Eligible to participate in Medicare
- 6+ months of Nurse Practitioner experience
- Experience managing chronic conditions
- Comfortable using clinical technology
- Nursing home care experience
- Geriatric or palliative care experience
- Pay: $120K–$136K/year
- Medical, dental, vision; 90% individual premiums covered
- Dependent coverage at 70%
- Up to 160 PTO hours plus 10 holidays
