Responsibilities:
- Provide in-home physical therapy services to patients across the Tri-County area.
- Evaluate, plan, and implement personalized treatment plans.
- Collaborate with interdisciplinary team members to ensure optimal patient outcomes.
- Educate patients and families on care plans and safe practices.
Requirements:
- Current Physical Therapist (PT) license in South Carolina
- Prior experience in home health (preferred but not required)
- Reliable transportation and valid driver's license
- Strong communication and clinical skills
- Compassionate, self-motivated, and team-oriented
- Proficient and timely with EMR point of care documentation
