Personal Care Aide / HHA / CNA – 12-Hour Shifts – Immediate Start
Job Overview
CH Family Care Partners LLC is seeking dependable, compassionate, and experienced Personal Care Aides, Home Health Aides (HHAs), and Certified Nursing Assistants (CNAs) for immediate in-home care opportunities.
We are looking for caregivers who are comfortable providing hands-on personal care, mobility assistance, repositioning, and transfers. Candidates should have experience supporting individuals who require a higher level of physical assistance with activities of daily living.
This role is ideal for caregivers who are comfortable working longer shifts and providing consistent, respectful, safety-focused support in a private home setting.
Responsibilities
- Assist with bathing, dressing, grooming, and personal hygiene
- Provide toileting and continence assistance
- Assist with safe transfers between bed, chair, wheelchair, and bathroom
- Assist with repositioning and mobility throughout the home
- Provide fall-prevention and safety monitoring
- Assist with walking and mobility devices as appropriate
- Provide companionship and emotional support
- Assist with meal preparation and light housekeeping related to client care
- Follow the individualized care plan
- Complete required visit documentation
- Communicate concerns or changes in the client’s condition to agency management
- Maintain professionalism, dignity, and respect at all times
Physical Requirements
- Must be comfortable providing hands-on mobility and transfer assistance
- Ability to safely lift approximately 50 pounds independently
- Ability to assist with heavier transfers or repositioning using proper techniques, equipment, or additional assistance when required
- Ability to stand, bend, reach, walk, and assist clients for extended periods
- Knowledge of safe body mechanics and transfer techniques strongly preferred
Qualifications
- Previous experience as a Personal Care Aide, HHA, CNA, caregiver, or similar role preferred
- CNA, HHA, PCA/PHCAST, or related caregiver training is a plus
- Experience with bathing, toileting, transfers, and mobility assistance strongly preferred
- Experience caring for older adults or individuals requiring higher levels of assistance
- Dependable, patient, respectful, and compassionate
- Strong communication skills
- Ability to follow care plans and document services accurately
- Reliable transportation to client locations
- Must be able to complete required background screening and employment documentation

- Additional ongoing assignments may be available as the agency continues to grow

Job Type:
Full-time / Part-time / Per diem opportunities available
Immediate availability is strongly preferred.
Pay: $18.00 - $19.00 per hour
