Job Overview
We are seeking a dedicated and compassionate Home Health Aide & Caregiver to join our team. In this vital role, you will provide essential support to clients in their homes, assisting with daily activities, medical needs, and overall well-being. The ideal candidate will possess a strong commitment to patient care, excellent communication skills, and experience working with diverse populations including seniors, individuals with disabilities, and those requiring specialized memory or dementia care.
Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, and temperature accurately using electronic health record (EHR) systems
- Administer medications and manage medication schedules in compliance with healthcare protocols and HIPAA regulations
- Support patients with specialized needs such as Alzheimer’s care, dementia care, hospice care, and catheter management
- Operate medical equipment including Hoyer lifts and assistive devices safely and effectively
- Maintain detailed medical documentation and ensure all records are accurate and up-to-date
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients
- Collaborate with healthcare professionals to coordinate patient care plans and update medical records regularly
- Provide emotional support and companionship to promote mental well-being
- Follow infection control procedures diligently to ensure client safety at all times
Experience
- Prior experience in home health or community care settings is highly preferred
- Experience working with individuals with developmental disabilities or long-term care needs is advantageous
- Knowledge of medical terminology, EHR/EHR systems, and HIPAA compliance required
- Demonstrated ability to perform patient observation accurately and respond appropriately to changing conditions
- Certification in CPR is mandatory; additional certifications such as hospice care or Alzheimer’s/dementia training are a plus
- Experience operating medical equipment like Hoyer lifts or administering medications enhances candidacy
- Previous work in assisted living or nursing home environments is beneficial
