What you'll do Provide patient-centered care across the full arc — from MCI to advanced dementia — as a trusted clinical partner through diagnosis, progression, and care transitions:
- Conduct cognitive evaluations, order/interpret diagnostics, and deliver diagnoses with clarity and compassion
- Build and manage individualized care plans, partnering with caregivers on behavioral and symptom management
- Assess function and safety (ADLs, mobility, fall risk) and recommend supportive services
- Coordinate across primary care, neurology, psychiatry, and community resources as clinical escalation point
- Educate and support caregivers through disease progression and crisis moments
- Lead advance care planning — goals-of-care conversations, directives, and proactive crisis prevention
Qualifications
- Active, unencumbered RN and APRN licensure
- 3+ years in geriatrics, neurology, dementia care, or related field
- PMHNP, AGNP, GS-C, or similar certification preferred
- Skilled at longitudinal care, caregiver dynamics, and end-of-life conversations
