Fall River Health Services is seeking a compassionate, skilled Licensed Practical Nurse (LPN) to join our Acute Care team in Hot Springs, SD. In this vital role, you will provide direct patient care under the supervision of a Registered Nurse (RN) and serve as an essential communication link between patients and the care team. As a Licensed Practical Nurse, you will:

Provide direct nursing care under the supervision of an RN, including personal hygiene care and support of daily patient needs.

Participate in the formulation, implementation, and updating of patient care plans.

Administer medications and treatments as ordered, including respiratory therapy modalities, and document accurately.

Apply and enforce infection control and safety policies to maintain a clean, safe care environment.

Use sound judgment to organize time and prioritize care effectively.
-
Monitor patient conditions, accurately observe changes, and report concerns or incidents promptly to the RN.

Respond appropriately in stressful, unusual, or emergency situations, taking action within scope of practice.

Provide patient and family education during hospitalization and upon discharge.

Maintain professional communication and build positive working relationships with patients, families, and staff.

Support the learning environment by assisting with orientation and continued training of new staff and students.

Maintain clinical excellence through participation in continuing education, applying and sharing new skills and knowledge.

Deliver nursing care using the nursing process, following facility policies, procedures, and standards in compliance with the South Dakota Nurse Practice Act.

Maintain confidentiality and uphold patient privacy and organizational trust.

Perform other duties as assigned based on facility needs.

Strong communication skills, including timely reporting of patient changes to the RN.

Education/Experience/Licensure
- Graduate of approved Licensed Practical Nurse Program with current licensure.
- CPR certification is required.
