Job Summary
We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our healthcare team. This role is essential in providing specialized care for patients with complex wound management needs. The ideal candidate will possess a strong clinical background and demonstrate expertise in patient assessment, treatment planning, and wound care techniques. The Wound Care Nurse Practitioner will work closely with multidisciplinary teams to ensure optimal patient outcomes.
Responsibilities
- Conduct comprehensive patient assessments to evaluate wounds and develop individualized care plans.
- Perform advanced wound care procedures, including but not limited to debridement, dressing changes, and catheter care.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and treatment strategies.
- Educate patients and their families on wound care management, prevention strategies, and overall health maintenance.
- Document all patient interactions, treatments, and progress in accordance with healthcare regulations and facility policies.
- Stay current with advancements in wound care practices and participate in ongoing professional development.
- Provide support for patients with developmental disabilities as needed, ensuring compassionate and effective care delivery.
Qualifications – Wound Care Nurse Practitioner
- Licensure & Certification:
- Current and unrestricted Nurse Practitioner (NP) license in the state of practice
- National Board Certification as an NP (AGNP, FNP, or equivalent)
- Valid and current DEA license (if required)
- Certification in Wound Care (WCC, CWCN, CWS, or similar) preferred or willingness to obtain within 6 months of hire
- BLS certification (required); ACLS certification (preferred)
- Experience:
- Minimum of 1 year of clinical experience as a Nurse Practitioner required
- At least 6 months of experience in wound care, dermatology, surgical, or geriatric settings preferred
- Familiarity with electronic health records (EHR) and documentation standards
- Skills & Competencies:
- Strong clinical assessment and decision-making skills
- Excellent patient communication and bedside manner
- Ability to work independently and as part of a multidisciplinary care team
- Comfortable providing care in skilled nursing facilities, long-term care settings, and in-home environments
- Understanding of Medicare/Medicaid and insurance documentation requirements
- Other Requirements:
- Valid driver's license and reliable transportation (as travel to multiple care sites may be required)
- Commitment to quality care, patient safety, and ethical practice
- Ability to manage a flexible schedule and maintain timely documentation

Pay: $120,000.00 - $250,000.00 per year
